Making a referral

    It is the responsibility of the referrer to have a conversation with the family to discuss their potential inclusion within the service.
    The referral will be acknowledged within 3 days.

    Post Acute Care

    It is the responsibility of the referrer to have a conversation with the family to discuss their potential inclusion within the service.

    How to make a referral: 

    Schoolcare Program

    The referral has three sections. One must be completed by the school, one by the family and one by the child/adolescent's treating physician. Link to Schoolcare Program referral. The referral is valid for a 12 month period

    NB: Children who attend Catholic schools must complete a separate referral form, which can be accessed from the Catholic Education Melbourne.

    Homecare Program

    To access the Homecare Program make a referral directly to the Complex Care Hub who will then distribute to the appropriate local Homecare services provider. Referrals can be received from either within the hospital setting or from the community. Referrals must be supported by a confirmed funding source prior to consideration for acceptance. All referrals undergo a preliminary assessment completed by a Clinical Nurse Consultant to determine their appropriateness for the Homecare Program and to clarify the child's care needs. This is done in conjunction with the referrer, the child's parents and primary medical practitioner.
