Joint RCH and adult service transition clinics
The Royal Children's Hospital (RCH) works collaboratively with many adult health services across Victoria and offers the option of joint RCH and adult service transition clinics. The RCH Transition Support Service currently runs more than 70 joint transition clinics annually to support young people and families in meeting with their new adult health care teams before being discharged from the RCH.
