Transition Support Service

Differences in services

  • The adult health care system is very different to the paediatric health care settiing in a similar way that secondary school is very different to university or the workforce. Depending on the young person's ability, there may be an expectation that the young people has developed self-management skills. Some of these may include taking responsibility for medications, filling prescriptions, attending appointments independently if appropriate and communicating important information to their health care team/s.

For more information about these differences please see the  Differences in Care: paediatric to adult health care fact sheet.
