Goals of patient care: Key discussions and decisions

This is intended as a brief overview of the progress of discussions with child/caregiver(s).
As ACP often requires multiple discussions to achieve clear consensus of plans with the family, it is useful for there to be documentation, particularly of ongoing discussion regarding resuscitation status.
This page should hold a summary of changes in child’s status, ‘trigger’ events, and the decisions that arise from these events. Reporting the family’s own words may be appropriate.
