The "Stratify JCV request form" has been replaced by an online registration process. All samples will be registered by a member of the Neurology team or Day Medical team electronically via the Unilabs website. A print out of the registered JC Virus test will be provided to labs along with the JC Virus tube & kit. A print out of the JC Virus Antibody Stratify lab pathology form must accompany the JC Virus sample kit to the lab department.

LABORATORY NOTES:
1. Allow sample to clot for 30-60 minutes post collection.

2. Centrifuge sample for 10 minutes at 3500rpm (2000g).

3. Aliquot and store serum at -70oC.
Send to testing laboratory on dry ice. Do not pack dry ice in a sealed container. use appropriate PPE.
