Adolescent transition is a process where you, your doctors and other health care professionals and your parents/carers work together to determine the best place for you to transfer to for your adult medical care. This is an important process for young people as it helps you feel ready to transfer to an adult health care service by identifying early on the skills and information that are relevant to transferring hospitals and navigating a new health service.
The RCH Adolescent Transition Education Program has been developed to provide both young people and their parents with information about different areas of transition. Topics include basic information about transition, self-management tips, health and well-being issues for young people, resources and links for school or work support, as well as links to other services.
