Contact Kids Connect (Primary Care Liaison)

Please contact Kids Connect if:

• You have recommendations or feedback that may help us improve community-hospital integration.
• You have any problems with communication (ie. discharge summaries, out-patient appointments, contacting a clinician etc).
• There is any other way we can help.

Kids Connect is the RCH GP Liaison / Primary Care Liaison program.
