Welcome to RCH @ Home

RCH @ Home is a team that organises, co-ordinates and provides excellence in paediatric healthcare as well as other support services in the home and in the community for children between 0-18 years.

These services can be provided directly to the child and family and to the community support agencies involved with caring for the child at home.

The services provided fall into five broad categories based on the anticipated duration of support: Hospital in the Home (HITH)Post Acute Care (PAC)Family Choice Program (FCP)Homecare Program and Schoolcare Program.

 

Being Part of RCH @ Home

Making a Referral

All referrals to Hospital in the Home, Post Acute Care, Homecare Progra, and the Family Choice Program are made via the Intake Liaison Nurse. Referrals should be made on the RCH@Home referral form.

Time of Visits

If you are part of our short-term programs (Hospital in the Home and Post Acute Care), then you can expect home visits from RCH @ Home Nursing staff. Our nursing staff will contact the family daily prior to the visit to confirm a timeframe they will visit within.

An exact time cannot be provided due to previous visits unexpectedly taking longer than planned or due to being held up in traffic.

If you are not going to be home during the period of time the nurse has said they will visit within, you will need to contact the on-call nurse on 9345 4770.

Attending Outpatients Appointments

On the day you are attending the RCH for outpatient appointments, the RCH@Home team will visit you in the Outpatient department.

Please bring your child’s folder, supplies and medications to every outpatient appointment.

Please request that medical staff contact the RCH@Home team with an update.

Contact the on-call nurse when you arrive at the RCH to arrange when and where a nurse can meet you.

Outsourced Patients

If your child’s care is provided by an external service provider you can still contact the RCH@Home on-call nurse on 9345 4770.
