How to refer children to Home and Community Care
This program covers activities previously known as Hospital in the Home, Post Acute Care, Homecare and the Family Choice Program.
Liaison nurses have been appointed to work with all staff to identify children and families who may require our services. These staff have extensive paediatric nursing experience as well as detailed knowledge of hospital and community services. They are available to assist with discharge planning and will work with staff to develop an appropriate package of home support.
