Ethics forms
The RCH Ethics and Human Research Committee accepts the Victorian Dept of Health's 'Common Application Form' for all ethics submissions. It is important to note that the RCH does not use the common participant information and consent forms as it has developed its own templates which are more suited to a paediatric environment. Please contact the RCH Ethics and Research Office for assistance with what forms researchers need to complete, and how to complete them.
